「性別焦慮症」（Gender dysphoria）是《精神疾病診斷與統計手冊》（Diagnostic and Statistical Manual of Mental Disorders）（DSM）中列出的一個診斷種類（diagnostic category）。【1】它首次作為一種心理學診斷被列入2013年問世的《精神疾病診斷與統計手冊》第五版（DSM-5）中。此後，第五版經歷了修訂，並在今年重新發佈。
關於性別焦慮症是否應該被納入《精神疾病診斷與統計手冊》一直存在爭議。有些人認為，為此創立一個診斷種類，會讓那些認定自己為跨性別者（transgender）的人將自己的情況歸於病態（病理化），而且在美國文化中，這種診斷只在跨性別者生理學手術的保險報銷中才有必要。【2】這篇解釋文將描述性別焦慮症的診斷以及有關診斷和治療的問題，特別是在兒童身上。
術語的定義
- （心理）性別（Gender）「指的是與生理性別相關聯的心理學和文化特徵。它是一個心理學概念和社會學術語，而不是一個生物學術語。」【3】
- 焦慮症（Dysphoria）在詞典中被定義為「一種不滿意、憂慮、不安或煩躁的狀態。」【4】
- 性別焦慮症（Gender dysphoria）（GD）是《精神疾病診斷與統計手冊》中的一個臨床術語，用於描述兒童和成人所經歷的心理狀況，其特點是他們體驗到的（心理）性別和與其生理性別相關聯的性別之間存在不一致。他們經常表達的信念是認為自己應該是異性。【5】
性別焦慮症的診斷標準
- 性別焦慮症的標準明確規定，只需經過6個月的觀察期就可以做出診斷。【6】
- 奇怪的是，性別焦慮症診斷的觀察期對兒童和成人都是一樣的。
- 一旦兒童的生理性別和他們對另一性別的體驗之間的不一致的感覺被表達出來，就會通過標記至少六個其他標準來做出診斷。
- 說來也奇怪，這些標準中有相當一部分類似於許多西方的刻板印象和傳統的性別角色。比如說：
- 標準3.強烈偏好虛構遊戲或幻想遊戲中的跨性別（cross-gender）角色
- 標準4.強烈偏好常規情況下由另一性別使用或參與的玩具、遊戲或活動
- 標準5.強烈偏好另一性別的玩伴【7】（此處引用《精神疾病診斷與統計手冊》標準的完整目錄。【8】）
確定性別焦慮症的診斷
- 沒有生理測試（即實驗室工作、影像學）可以確定焦慮症狀的程度。【9】
- 沒有心理測試可以預測誰會對跨性別手術感到後悔。
- 沒有心理測試可以預測性別焦慮症將持續多久。
- 沒有心理測試可以預測誰將因跨性別手術而受到所謂的好處或傷害。
- 沒有標準化的措施來評估性別焦慮症的病原學（etiology）或不同根源。
- 沒有測試可以檢測出幾十年前因性別焦慮症進行治療的大部分生理男性與目前舉止類同的表現出症狀的大部分生理女性之間的差異。
- 沒有心理測試可以區分所有種類的跨性別經驗（例如，非二元性別，跨性別男性，跨性別女性）和所有上述的心理和生理測試。
- 對於性別焦慮症，唯一可以預測的成果是，在許多情況下，如果不加理會，它將在兒童身上得到解決。
- 長大後擺脫性別焦慮症的兒童數量，即「斷念」（desistance）率，在生理男性中的比率為70%至97.8%，在生理女性中為50%至88%。【10】
將性別焦慮症與其他被《精神疾病診斷與統計手冊》解釋為成熟期的診斷進行比較
- 對兒童的成熟過程做出解釋，在其他《精神疾病診斷與統計手冊》第五版的診斷中得到了普遍認可和支持，這些診斷包括觀察一種穩定的行為、情感和思想模式。【11】
- 在進行這些診斷時，標準要求兒童呈現出症狀的時間要比成人更長。建議在18歲以後進行診斷。【12】
- 在少數情況下，如果診斷發生在18歲之前，必須觀察一年的症狀，以確定穩定的模式（與六個月的性別焦慮症不同）。【13】
- 這些疾病的等待期與一個被廣泛接受的事實有關，即一個人的大腦在二十多歲時仍在繼續發育。【14】
- 大腦中的邊緣系統也被稱為我們情緒的所在地，與情緒處理、學習和記憶有關。
- 邊緣系統在一個人的心理健康中起著重要作用，並且需要最長的時間來達到結構性常態規範。許多人在20多歲時才達到成熟的常態規範。
一個好的診斷評估涉及到排除其他導致性別焦慮症的原因
- 好的執業醫師會花時間並徹底仔細地詢問兒童和主要照料者，以排除其他診斷以及可能促成呈現出問題的因素。比如：
- 有充分資料顯示，那些呈現出性別焦慮症的人也可能經歷以下一些問題：
- 兒童時期的軀體虐待、性虐待和情感虐待以及忽視【16】【17】
- 自閉症【18】
- 受同齡人及社交媒體網站的影響【19】
來自醫療和心理建設機構的治療建議
- 當涉及到《精神疾病診斷與統計手冊》中列出的其他診斷時，研究人員和理論家被鼓勵開發治療方案。臨床醫生被鼓勵去讓他們的客戶意識到不同的治療方法可選擇，以及憑藉他們所受的培訓和專業知識，他們自己能夠提供什麼。例如，如果你在經歷某個創傷事件後去治療，你可以找到一個能夠為你提供認知療法、藥物治療、眼動脫敏再處理療法（EMDR）、暴露療法等療法的治療師。
- 當涉及到性別焦慮症診斷時，無法提供多種治療方案來減少症狀。
- 關於性別焦慮症能為你提供的唯一治療方案是進行社交方面和醫學方面的過渡轉變。
- 這種選擇會導致將改動和/或移除健康器官和身體系統的生理學手術以治療心理疾病。
- 使用青春期阻斷藥物會導致一系列兒童健康問題，包括絕育、骨密度降低、認知障礙、身體脂肪比例和體重指數（body mass index）增加、去脂體重（lean body mass）減少和動脈高血壓（lean body mass）。【20】【21】【22】
- 跨性別手術是已知的治療心理疾病的最具侵入性的生理做法。
- 這些手術只有最少的科學證據支持其使用。
- 在提供跨性別生理手術時，無視性別焦慮症診斷標準中表明的6個月觀察期。
- 像計劃生育協會（Planned Parenthood）這樣的團體在初次就診時就發放跨性別激素，而沒有進行心理健康評估或征得父母同意。【23】
- 對於誤診和錯誤分發青春期阻斷劑和跨性別激素以及對未成年人進行外科手術的行為，沒有聯邦把關機制來追究從業者或提供者的責任（除非一個州已經追求立法來限制對未成年人進行手術【24】）。
- 治療應該具有特點：
- 最起碼，應該對兒童及其照料者的心理狀況歷史進行詳細評估。
- 評估應該說明臨床醫生如何排除可能造成性別焦慮症表現的其他已知因素（比如創傷、自閉症、社交傳染）。
- 這些其他因素應在治療計畫中得到解決。
- 研究人員和臨床醫生應擺脫只要求一種治療方案的跨性別者意識形態的束縛。
